Add ignored patterns for:
- Test and coverage reports (coverage/, .lcov, test-results/)
- Playwright test artifacts (playwright-report/, blob-report/)
- Quality check state files (.quality/, .qualityrc-cache/)

Prevents auto-generated files from being committed to git
